[Legacy] release forms

What have others done for release forms for community property?  We want
to get permission to digitize Women's Club Scrapbook that they loaned to
the library and would appreciate not reinventing the release form wheel
so to speak.  There will be other donated materials as well from the
community.

What has been done for newspaper clippings? is the citation in Dublin
Core sufficient, or will a permission form from the newspaper publisher
be necessary for digital publishing?  Please send files if you have
them.
