I think it looks fine.  The only thing I would change is the wording on question 2 to read:
 
If yes, how many staff members are you anticipating being able to send?

VIP Committee Members-

 

At the June 6th meeting we decided to survey libraries who have sent staff to the VIP in the past to assess their intentions for sending staff in 2009.  A draft of the survey document is attached.  

 

Please review and provide any comments, suggestions, edits to me as soon as you can.  When we have reached a final version TBLC will make the survey available electronically using a tool called Survey Monkey.  We will email the directors of libraries who have sent staff in the past and ask them to complete the survey online.

 

Thank you for your help.  

 

Please have your comments to me before June 27th.  We would like to survey in July so we can have adequate time for feedback before the end of the summer.
